Building a better approach to healthcare

A new model of integrated care

We believe that integration is the future of healthcare, with a seamless passage for patients from primary through to specialist care and co-location of diagnostic and treatment facilities.

At the Integrated Specialist Healthcare Education and Research Foundation (ISHCERF), we support the development of innovative models of integrated care in Australia. And we invest directly back into research, technology and education, so that we can apply what we learn today for a better tomorrow.

It’s about improving health services through education and by better linking hospital, specialist, GP and diagnostic services. It’s about giving back without expecting return. And that means better outcomes.

What is integrated healthcare?

The World Health Organisation defines integrated healthcare as:

“…bringing together inputs, delivery, management and organization of services related to diagnosis, treatment, care, rehabilitation and health promotion. Integration is a means to improve services in relation to access, quality, user satisfaction and efficiency.”

Put simply, it’s about joining together and putting systems in place to ensure you receive the care you need, from where you need it, when you need it.

How we help

  • Provide geographic co-location of services to provide patients with better access
  • Integrate diagnostics and procedures
  • Link all sectors in healthcare in partnership
  • Use technological innovation and electronic medical records to provide seamless communication between carers
  • Vertically integrate specialists with primary care and ambulatory care
